Having trouble setting up my yahoo email on my Lg Prada Phone.?

I can send emails but I'm having trouble recieving them. When I set up my yahoo account It asked for an incoming and outgoing mail server?

I to am having trouble recieving mail. When I bring up my Sent box and look at one of my sent messages it has a completely different name by the "From". And I have not heard back from any of my friends for a little over 2 weeks and I am getting really worried.
